The primary objective of the LTSR is to provide its residents/consumers with the individualized services required to maintain them safely and productively in a community based setting. The goal of the program is to insure that the residents/consumers have acquired, upon discharge, the necessary skills for independent living.
This position will care for program residents/consumers, following physician instructions and facility procedures. Monitor resident/consumer physical and emotional condition and report to medical and program administrative staff. Participate in treatment programs, help with personal hygiene, and supervise administration of oral medications.
Duties include:
1. Supervise resident/consumer self-administration of prescribed medications.
2. Provide direct resident/consumer care.
3. Assist in the scheduling of resident/consumer medical and dental appointments and follow-up care.
4. Accompany residents/consumers to and from medical and dental treatments, shopping trips, and religious and recreational events.
5. Maintain accurate, detailed reports and records.
6. Participate in recreational activities with residents/consumers, including card games, sports, or television viewing.
7. Provide clinical services to resident/consumers under supervision of licensed clinical staff.
8. Supervise meals and food preparation
9. Monitor all aspects of resident/consumer care, including diet and physical activity.
10. Provide residents/consumers with assistance in bathing, dressing, and grooming, demonstrating these skills as necessary.
11. Organize, supervise, and encourage residents/consumers participation in social, educational, and recreational activities.
12. Monitor, record and report symptoms and changes in resident’s/consumer’s condition.
13. Work as part of a team that may include psychiatrists, psychologists, psychiatric nurses and social workers.
14. Report to the psychiatrist and other program team members to assess, plan, implement and evaluate resident/consumer treatment plans.
15. Attend staff meetings, in-services, conferences and other job-related training/educational sessions as scheduled.
The following knowledge/skills are required:
Knowledge of the principles and processes required for providing resident/consumer and personal services. This includes resident/consumer needs assessment, meeting quality standards for services, and evaluation of resident/consumer satisfaction.
Knowledge of human behavior; individual differences in ability, personality, and interest; learning and motivation; and the assessment and treatment of behavioral and affective disorders.
The State Regulation expects; at least 12 college credits in mental health / a related field OR a Bachelors degree and at least 6 months employed in a mental health job or related field.
