What you can expect:
Reporting to the Clinical Supervisor, the Case Manager provides case management services, coordination of services and resources to assigned individuals served and their families who have a severe emotional disturbance, clients with a severe and persistent mental illness, and individuals with a co-occurring SED or SPMI disorder.
How you will make an impact:
- Coordinate person centered plans for assigned clientele, including arranging for meeting times, invitations, and completion of the final plan and accompanying documents (i.e. crisis plans, advance directives, referrals for behavioral, communication occupational assessments and therapies).
- Provide 175 units per month of face to face treatment, as outlined in person-centered plan, including core competencies of Case Management: assessment, planning, linking, coordinating and monitoring.
- Provide services using resiliency, recovery focused, evidence based or promising practice treatment modality; person first language and Gentle Teaching methodology. Services are provided in a trauma informed process.
- Coordinate and monitor community-based living arrangements (AFC, Specialized Residential, Supported Independent Living, Independent Living) for individuals served.
- Documentation completed is timely, strengths based, utilizes recovery methods and measures consumer progress and satisfaction with services.
- Coordinate care with other external organizations as necessary (MDHHS, courts, law enforcement, social security, transportation, community resources, medical providers, etc.), including jail diversionary activities as appropriate.
- Provide or arrange for crisis intervention services when needed, including inpatient referral.
- Coordinate care with primary care physicians/pediatricians and Qualified Health Plans.
- Attend and participate in a variety of in-service training to maintain and improve job skills, including all agency-required in-service training. Attend (on time) and participate in designated staff meetings and clinical (treatment) meetings.
- Will attend a minimum of 24 hours of in-service training (annually during calendar year) related to provision of services to children (CDTC certification requirement for all staff who work with MI children)
- Maintain licensure/registration as appropriate to profession and job requirement
- Participates in scheduled supervision sessions with immediate supervisor
- Will maintain a minimum of 80% overall satisfaction as measured by designated consumer satisfaction rating scales (during designated measurement periods
- Adhere to all policies and procedures of MDHHS, CMH, SWMBH, CARF, Medicaid and other regulating bodies as designated.
- Maintains strict confidentiality and advocates for the rights of individuals served
- Emergency services as needed or requested by supervisor
- Other duties as assigned
Minimum Education & Experience Requirements:
- Bachelor’s or master’s degree in social work, Sociology, Counseling, Psychology
- Ability to obtain State of Michigan Licensure
- QMHP/QIDP/CMDP preferred
