- JOB SUMMARY
- The Ministry Transportation Facilitator/Mobile unit driver is responsible for safely transporting patients utilizing the HCS Van Ministry to and from their medical appointments at KDMC. This curb-to-curb service provides free transportation for patients who have limited resources and reside within a 25-mile radius of the OLBH campus. Patients scheduled for outpatient services such as outpatient testing, same day surgery, chemotherapy and radiation treatment, cardiac and pulmonary rehabilitation, physical therapy, occupational therapy, speech therapy, campus physician offices, and King’s Daughters Primary Care Centers may use this service and is responsible for driving and maintaining the Mobile Health Unit on a PRN basis. Ensures the safety of the vehicle and assists the team during scheduled clinic sessions.
DUTIES AND ESSENTIAL FUNCTIONS
- Overall performance rating of below indicated essential job duties:
- Transport of patients to medical appointments as scheduled.
- Maintains safety of patients being transported through the use of safety belts and special accommodations (i.e. wheelchair lift, walker, cane, portable oxygen).
- Operates all equipment on vehicle including wheelchair lift, wheelchair tie-downs, and cellular phone. .
- Completes written daily schedules and logs to efficiently transport patients.
- Performs routine safety and maintenance checks on vehicle and is responsible for keeping the vehicle clean.
- Maintains current CPR/First Aid certification.
- Demonstrates a safe driving record and maintains a valid Kentucky driver’s license in good standing.
- Assists patient on and off the Mobile Unit. Provides close patient monitoring, as directed, and recognizes and immediately reports, within the limits of the Driver/Coordinator’s field of knowledge any changes in the medical or mental status of the patient.
- Provides a high level of customer service, assisting customers in a friendly and helpful way, representing the Medical Center in a positive responsible manner.
- Maintains a positive working relationship with mammography technicians, occupational health staff, health department personnel, etc.
- Completes work and documentation with accuracy within the established time frame.
- Performs general cleaning, organization and maintenance of equipment/working areas as assigned. Maintains the work area to reduce the likelihood of safety hazards and to enhance its general appearance.
- Ensures that the Mobile Unit is serviced as required and keeps logs of such maintenance.
- Cross trains to assist with screenings, registration, and other appropriate clinical or non-clinical duties.
- Maintains Mobile Health Unit’s stock at all times in preparation of next day usage. Demonstrates good cost-conscious judgment in the use of supplies. Maintains medical center property, supplies and equipment in a manner that demonstrates ownership and accountability.
- Assists other Community Services team members in staffing and organizing community health events. Takes a leadership role in the department for all issues relating to community health needs. May be required to independently staff programs or projects
- Completes Occupational Medicine testing after validation such as urine dip sticks, urine drug screens or any other applicable testing

- EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS
- Minimum requirement:
- High school/GED diploma.
- Valid driver’s license and an acceptable driving record with no major violations.
- Three (3) years driving experience
- Commercial Driver's License (Class B)
- Preferred qualifications:
- Two years Healthcare experience
- Valid Commercial Driver's License (CDL; Class A)

- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception and ability to adjust focus.
