Clark County (Indiana) Government and the Clark County Health Department is seeking qualified applicants for the position of Mobile Integrated Health Coordinator 2 - EMT. This is a part-time grant funded position.
The Mobile Integrated Health Coordinator delivers medical care for patients in the home or in a mobile environment. Services provided by the coordinator in the home setting include basic health screenings, vaccine administration, telehealth assistance, wound care, and limited general ER/hospitalization follow-up care.
In summary, the coordinator will work with clinical partners such as local hospitals and Federally Qualified Health Centers to provide care in the home when essential. The coordinator will utilize approved treatment plans certified by the Clark County Health Officer in conjunction with the primary care designee of the patient. The candidate will ensure appropriate charting of patient care and share outcomes with the supporting medical provider.
The expectation for treatments includes:
Substance Use Disorder follow-up consultation after discharge from Emergency Medical Services
Health screenings or assessments including blood pressure, glucose, and lip screenings when indicated
Evaluation of safety in the home which includes safe sleep practices and concepts for new parents, postpartum depression evaluation, fall/trip hazard and nutrition needs
Health Education, coaching, assistance for social support
Training and other duties/responsibilities include: CPR and First Aide Training, work in a safe manner in accordance with OSH/IOSHA, adhere to federal state, and local laws, utilize emergency medicine equipment, prepare weekly and/or monthly reports for IDH or other community partners, communicate monthly with community partners such as hospitals, EMS, FQHC's, observe and adhere to federal, state and local rules/regulations governing Personal Health Information
Education and Experience: Licensed Emergency Medical Technician in the State of Indiana; Three (3) Years of Experience combined EMT/PARAMEDIC practice
Other requirements include: must be able to work as a team member and independently, in and out of state travel and routine travel, assist with public health programs including immunizations, must be able to lift assist patients, must be able to bend, stoop, kneel, crawl or hike, and must possess a valid driver's license
