Summary:
Provides assessment, planning, counseling and education to patients.
Responsibilities:
Responsible for comprehensive, individualized treatment planning on a timely basis including completing initial treatment plans with patients, treatment plan reviews and discharge planning.Initiates Baker Acts as needed; communicates with patients and their caregivers to facilitate voluntary crisis stabilization admissions as needed. Ensures that all legal paperwork is obtained, processed and completed, and included in patient charts ( i.e. guardianship, baker-act, firearm laws).Implements evidence based (i.e. Motivational Interviewing) patient & family counseling and education, case management & safe discharge planning.Conducts evidence based group therapy to patients to address psychological, emotional or aftercare needs (i.e. Trauma, Co-occurring disorders, seeking safety, wrap) and completes clinical documentation within 24 hours of service delivery.Supervises interns and students as needed.Completes psychosocial evaluations to ascertain patient's psychological, emotional and aftercare needs. Identifies and refers patients to appropriate community resources for follow up care.Provides primary care coordination to a caseload of patients including integrating treatment with multidisciplinary members of care team, conducting ongoing assessment of patients' response to treatment using validating tools, and providing behavioral health counseling services including individual, family and group therapy. Updates and discusses patient strengths and needs in Multidisciplinary treatment team meetings.Conducts suicide risk screenings, suicide risk assessments and safety planning with patients following department policies and procedures.

Must communicate effectively and respectfully with patients, families, and all levels of staff and management. Required to provide conscientious commitment to assigned work, work schedule, honesty, teamwork, assisting others to bring about positive outcomes. Must be able to recognize and manages risks, resolve issues and problems as soon as they arise, report and document adverse events and reportable conditions. In all respects, verbally and behaviorally, must support Memorial Healthcare System's mission and values.Must demonstrate the ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge. Required to identify and intervene for suspected abuse including sensitivity to age specific concerns; physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; and caretaker/institutional abuse for senior adults. Must be able to manage crisis situations, demonstrating ability to quickly appraise the crisis situation and select appropriate intervention. Able to document direct service in patient's medical record after each significant encounter. Must demonstrate proficiency in use of the electronic medical record. Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement. Required Work Experience: One (1) year social work experience in a health care setting.
