40hr Days, 7a-3:30p, 1 weekend per month, E/O Holiday.
Required Exp: 2 years + post MSW, Case Management, Brief Treatment, ICU, Critical Care.
Preferred Exp: Acute Care, Housing Experience, Substance Use Disorder, Grief and Bereavement , Cultural Responsiveness, Crisis Intervention
Required Certs: Licensed at the Masters Level with Masters in Social Work degree, BLS
Dept Details: Inpatient: Surgical ICU, and Inter,te Care. Patients are referred for social work consults 15-20 patients per day # of beds: 30-40.
" This is a medical social work Client we are paired alongside nurse case managers who support with discharge planning.
" We assess social determinants of health by completing biopsychosocial assessments with the goal of increasing access to quality health care and reducing barriers to discharge.
" We provide patients and families with emotional and resource support by providing interventions that are strength-based, focused on the whole-person, trauma informed and span across the life cycle of our patients
" We collaborate with the multi-disciplinary teams on how the illness or injury may impact the patient s, family, employment and living situation.
" We promote health and reduce health inequities by advocating for, educating and empowering patients and families.
