Navigator RN - Primary Care at Providence

Description

The RN Care Navigator is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

Required Qualifications:

• Coursework/Training - Graduation from an accredited school of Nursing.
• Washington Registered Nurse License upon hire.
• National Provider BLS - American Heart Association upon hire.
• 2 years - Work experience in an RN role.

Preferred Qualifications:

• Bachelor's Degree - Nursing.
• Certification of diabetes education (CDE) upon hire.
• Experience managing complex patients in an ambulatory setting.
• Medical Home Model experience.
