100% FTE, Monday - Friday, 7:30am - 4:00pm. 1-2 weekends per year and 1 holiday every 3-4 years. Hours may vary based on the operational needs of the department. You will be working at University Hospital in Madison, WI.

We are seeking a Nurse Case Manager with:
- A broad clinical background with well-developed assessment and critical thinking skills.
- Understanding of medical necessity, level of care and patient status (inpatient vs outpatient).
- Knowledge of community resources such as transportation, skilled nursing facilities or home health agencies.
- The desire to ask questions and work within a collaborative team environment.
Education:
Minimum – Bachelor’s degree in Nursing. Equivalent combination of education and experience will be considered.
Preferred – Master’s degree in Nursing or healthcare related field.
Work Experience:
Minimum – Three (3) years of relevant clinical nursing experience.
Preferred – Recent experience as a hospital-based Nurse Case Manager.
Licenses and Certifications:
Minimum – RN license in the state of Wisconsin.
Preferred – ACMA certification as a Case Manager.
