Job Summary:
The Nurse Navigator will work with the Principal Investigators and Clinical Team Members of the Hartford HealthCare Headache Center on the Hartford HealthCare (HHC) Migraine Education and Management Program in the Workplace, the first US healthcare system-wide employee program to raise awareness and understanding of migraine, break down stigma, and facilitate diagnosis and management through expedited referral to the HHC Headache Center, with the goal of positively impacting workplace productivity and employee wellness.
The Nurse Navigator’s primary work location will be the headache clinic in West Hartford. Responsibilities include but are not limited to the following:
· Recruitment and screening of study participants via phone, virtual video, and in person. Clinical data entry into the patient charts and electronic medical record.
· Clinical data abstraction and entry: Collect clinical data from direct interaction with patients, surveys, patient charts, and electronic medical records (e.g., EPIC) and enter into databases for analysis.
· Assist with patient scheduling, obtaining vitals, collection of patient clinical information, and prior-authorization and appeal for medications, imaging, and procedures.
· Assist with monthly coaching phone calls to the patients.
· Assist with administrative responsibilities for our headache program
Qualifications
· Nurse certification (e.g. LPN, RN).
· Graduate of an accredited nursing school ASN Required, BSN preferred.
· BLS certification by the American Heart Association required prior to date of hire.
· Direct patient clinical experience.
· Strong attention to detail and ability to complete tasks in a timely manner.
· Preference given to candidates with direct clinical experience.
· Ability to provide support and empathic communication to patients in severe pain with a multitude of symptoms.
· Ability to work well within a group medical practice setting.
· Ability to be flexible with daily tasks.
