What You Will Do:

Ascension St. Vincent is looking for a PRN- Family Medicine for our Walk-In Clinic. 

 

This is a great opportunity for the right candidate to partner with Ascension Hazel Dell to provide medical services to patients.

 

Position Highlights:

• Walk-in Clinic 
• 2 providers

Responsibilities:

 

In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.

• Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunization and patient care.
• Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Licensure/Certification/Registration:

Required Credential(s):

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• One or more of the following required:
• Nurse Practitioner credentialed from the American Nurses Credentialing Center (ANCC) obtained within 6 Months (180 days) of hire date or job transfer date.
• Nurse Practitioner credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date.
• Nurse Practitioner credentialed from the National Certification Corporation (NCC) obtained within 6 Months (180 days) of hire date or job transfer date.
• Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.

Education:

• Master's degree of Nursing required.
