Title: Nurse Practitioner with Certified Diabetes Educator Credential

We are seeking a Nurse Practitioner with a Certified Diabetes Educator credential to join our endocrinology team in Rockville, MD. This remote position includes occasional office visits.

• Well-established private practice with 25 physicians, 7 nurse practitioners, 2 clinical psychologists, and a registered dietitian.
• Providers deliver comprehensive primary and multispecialty care, including dermatology, endocrinology, gastroenterology, pulmonology, rheumatology, sleep medicine, and mental health services.
• Fully integrated EMR system and robust referral base.

Position Details:

• Remote work with occasional office days.
• Part-time or full-time schedule with flexible hours; no evenings or weekends.

Responsibilities:

• Provide diabetes care, including patient education on management, lifestyle modification, and medication.
• Develop and adjust individualized care plans based on patient needs and progress.
• Conduct assessments, screenings, and follow-up visits to manage diabetes and related conditions.
• Collaborate with healthcare professionals to coordinate patient care and optimize outcomes.
• Educate patients and families on diabetes self-management, including blood glucose monitoring and insulin administration.
• Stay updated with the latest research and best practices in diabetes care.

Qualifications:

• Certified Diabetes Educator.
• 2+ years of experience required, preferably in diabetes management.
• Strong patient relationship skills, excellent bedside manner, and clinical proficiency.
• Ability to interpret data from glucose monitors and other diagnostic tools.
• Experience with developing and implementing diabetes management plans.
• Compliance with HIPAA regulations.
• State licensure and board certification required.
