- Job Summary
- In collaboration with a physician, diagnoses and treats certain acute or chronic health conditions in specific populations. Training is provided relevant to the population served, based on scope of care of the service assignment.

- Job Responsibilities
- Obtains patient history and perform assessments via observation, interview and examination. Orders, performs and interprets diagnostic studies. May be required to perform preventative health assessments, screenings, immunizations and care.
- Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
- Provides counseling and education of patients and their families/caregivers concerning preventative health, treatment options and community resources.
- Provides care in the context of promoting age and culture-appropriate informed choices, shared decision- making and the right to self-determination.
- Provides accurate and timely documentation of assessment and patient care plan.
- May coordinate the registration functions including collecting and entering demographic, insurance, employer information, and co-payments.
- Education
- Master's degree in Nursing required.
- Licensure/Certification/Registration
- Licensed as an Advanced Practice Registered Nurse (APRN) or other designation as required by state law. Certification by the American Nurses Credentialing Center (ANCC), the American Academy of Nurse Practitioners (AANP) or other accredited organization depending on specialty . Certified as a Basic Life Support (BLS) Healthcare Provider preferred.
- Work Experience
- None required. Experience in a specialty area preferred.

- Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
