Nurse Practitioner - Tandym

A Federally Qualified Health Center (FQHC) in New York is actively seeking a dedicated Nurse Practitioner (NP) to join their team. This role involves providing comprehensive primary care, managing chronic conditions, and collaborating with healthcare providers to improve health outcomes for a diverse patient population.

Responsibilities:

• Provide comprehensive primary care services, including diagnosis, treatment, and follow-up care for patients of all ages.
• Develop individualized care plans and manage chronic conditions such as diabetes, hypertension, and asthma.
• Educate patients and their families on health conditions, preventive measures, medications, and healthy lifestyle choices.

Qualifications:

• Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
• Board certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or other relevant NP certification.
• Experience in a community health, primary care, or FQHC setting.
