Join our team as an Advanced Practice Clinician (APC) - Nurse Practitioner

Location: Reading PA
Worksite: Senior Living Facilities. (2 total to visit)
Job Type: Contract 4-6 months depending on need.

We're seeking a dedicated provider to join our integrated care delivery program, focusing on our highest-risk health plan members in a facility setting. As an APC, you'll work collaboratively with an interdisciplinary team to provide comprehensive care, manage acute and chronic conditions, and conduct advanced care planning conversations.

Enjoy work-life balance with our flexible Monday to Friday schedule, 8am-5pm, and **NO** on-call, weekends, or holidays required. With a competitive pay rate of $65-$72 per hour, this role offers rewarding opportunities to make a difference in patients' lives.

Key Responsibilities:
- Conducting comprehensive assessments
- Managing acute and chronic medical and behavioral conditions
- Collaborating with the care team and ensuring accurate documentation
- Providing counseling, education, and advanced illness planning

Preferred Qualifications:
- 1+ year of clinical experience, preferably in a long-term care setting
- Proficiency in geriatrics, chronic illness, and end-of-life discussions
- Strong computer skills for medical documentation
