We are seeking a Nurse Practitioner to join our team; someone who wants to practice medicine with a focus on patient care at our established clinic in West Palm Beach, Florida.
ESSENTIAL RESPONSIBILITIES
Demonstrates both autonomy and proficiency in performance of comprehensive health assessments for the Medicare Advantage patient population.
Collaborates with member’s primary care physician to create a care plan focused on prevention, early detection and timely intervention.
Observes the scope of practice in the state in which he or she is licensed and working in.
Communicates with physicians and family members during transitions of care.
Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care.
EDUCATION & QUALIFICATIONS
Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) program focusing on adult medicine
Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employment
Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
Up-to-date CPR Certification and Basic Cardiac Life Support certification.
Strong organizational and prioritization skills.
Ability to use Electronic Medical Record (EMR) system – specifically ECW
Knowledge of Medical Advantage, HEIDS, MRA and ICD-10 Coding
Comfortable managing a primarily Medicare-aged patient population
Demonstrated ability to problem-solve complex and multifaceted situations.

You have both a legal and moral obligation to protect the privacy of our patients
In the course of your work, you will have access to confidential information regarding patients, and/or the practices' confidential business
It is essential that you refrain from any discussions regarding personal information about a patient, a patient's condition, a patient's finances, proprietary company information, personnel salaries, and/or the practices' confidential business with any third person
This includes, but is not limited to, other employees, your spouse, family or friends
CONFIDENTIALITY IS SO IMPORTANT THAT ANY BREACH OF THIS POLICY WILL BE CONSIDERED GROUNDS FOR TERMINATION
