About this role:

As an Advanced Practice Provider at our Heart Failure Clinic, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will work with a multidisciplinary team of doctors, case managers, nurses, dietitians, and pharmacists to identify patients who are at high risk for heart failure to help prevent unnecessary readmissions. Youll see approximately 6-8 patients per day in the inpatient and outpatient settings

• You will practice at St. George Regional Hospital
• Your hours will be Monday through Friday, 8am-5pm
• No call required

What youll bring:

• Unrestricted Utah NP or PA license
• Current Utah DEA certificate required prior to start date
• Current Basic Life Support Certification (BLS) for healthcare providers
• Ability to successfully complete Intermountain Healths credentialing process
• A minimum of three years of experience as an Advanced Practice Provider in Cardiology; experience in heart failure preferred.
