University of Minnesota Physicians is seeking aCertifiedNurse Practitioner orCertifiedPhysician Assistant to join the Physical Medicine and Rehabilitation (PM&R) team at the M Health Fairview Acute Rehabilitation Center (ARC) at the University of Minnesota Medical Center.
Practice Details:
In this position, the successful candidate will work collaboratively with experienced APPs and academic PM&R physicians within a highly specialized rehabilitation care team. Our team includes Nursing (including Certified Rehabilitation nurses and Nursing Assistants); Physical Therapists; Occupational Therapists; Respiratory Therapists; Speech-Language Pathologists; Social Workers, Psychologists; Care Coordinators, consulting providers and medical trainees who collectively identify, develop, and implement an appropriate plan of care. Our population includes a wide variety of patients, most frequently treating patients recovering from stroke, brain injury, spinal cord injury, other neurological conditions, amputation, transplant, or several other surgical and medical diagnoses who require three hours of therapy daily up to 6 times per week, with an average length of stay between 7-21 days.
The successful candidate will be responsible for delivering exceptional care to patients with complex medical conditions in our mission to support patients in their return to the community as freely and independently as possible.
Key responsibilities of this position include:
Monday – Friday, day shift
0.75 FTE with an expected work week consisting of approximately 30 hours
Consulting on hospital inpatients evaluating appropriateness for next level care including acute rehab setting
Performing daily rounds, admissions, discharges and consults and completing the proper documentation
Daily collaboration with physicians, physical therapists, occupational therapists, speech therapists, psychologists, care coordinators, dietitians, pharmacists, social workers, other consulting providers and medical trainees to ensure provision of the highest quality and efficient patient care. Multi-professional teamwork is an essential component of this work
Facilitates continuity of care across the continuum of services
Educates patients and families regarding disease process, interventions, discharge transitions, and postoperative and post-procedural care
Actively participate in staff education, research, process improvement initiatives, and administrative activities
Primary work intended for ARC and inpatient consults; however, there is a potential to add virtual and/or outpatient visits
Serve as a clinical expert and resource for the education of peers and other health professionals
Interpersonal and communication skills are required, as well as precise written and verbal handoffs to subsequent team members

Qualifications:
Graduate of an accredited Nurse Practitioner training program or a graduate of an accredited Physician Assistant program
Current MN RN and NP license through the Minnesota Board of Nursing or a current MN PA license through the MN Board of Medical Practice.
Certified through ANCC/AANP/ACNP or eligible for ANCC/AANP/ACNP national exam or through the NCCPA and meet threshold criteria/qualifications for Credentialing and Privileges

What will set you apart from other candidates:
Including a cover letter with your application describing your interest in physiatry or rehab medicine
Prior experience in Physiatry or a similar area of specialty
One or more years of experience within an inpatient hospital setting is preferred but not required.
Experience with medically complex patients and patients with rehab needs is preferred
