Nurse Practitioner Outpatient Spec Loc - Legacy Health

$112k to $167k yearly salary range (1.00 FTE)

• Opportunities for flexible FTE ranging from 0.80-1.00 FTE

Legacy is recruiting an Outpatient Advance Practice Practitioner (NP or PA) for our Business Health Services clinic at Woodburn Health Center, located at 1475 Mt Hood Ave, Woodburn, OR 97071.  Woodburn is a vibrant community located one hour south of the Portland Metro in the beautiful Willamette Valley.  Our clinic is conveniently located two miles from I-5 and a few blocks from 99E on Highway 211.

Legacy Medical Group Business Health Services provides occupational and business health services.  The providers and staff are dedicated to helping employers in the Mid-Willamette Valley and beyond maintain a safe, healthy, and drug-free workforce and are focused on delivering high-quality medical care with a philosophy of timely return to work.  The clinic provides treatment for job-related injuries along with custom medical screening, monitoring, and training services to clients to build and maintain a strong workforce. 

Providers in this clinic are currently Nurse Practitioners, all of whom have been a part of Legacy for several years. 

Qualification and Licensure Requirement:

• Graduate of Nurse Practitioner or Physician Assistant program.
• Minimum of 2-years’ experience.
• Current OR NP or PA license w/Prescriptive Privileges.
• Current BLS certification from the American Heart Association
• Willing to adhere to guidelines set forth in Legacy’s Values in Action
