Nurse Practitioner Job Description
Philadelphia FIGHT was initially founded in 1990 as a comprehensive AIDS service organization. Today we are a Federally Qualified Health Center (FQHC) with clinical sites that provide state-of-the-art culturally competent primary care, advocacy, social services, consumer education and outreach for patients in need regardless of HIV or insurance status.
The patients and staff at Philadelphia FIGHT are diverse and represent a broad spectrum of race, religion, gender identity, sexual orientation, HIV status, and physical ability. The function of the Nurse Practitioner is to provide primary care for patients whose care may be complicated by HIV, HCV, substance use disorder, and other vulnerabilities including race, ethnicity, language, immigration status, and LGBTQIA+ status at Philadelphia FIGHT's FQHC Community Health Centers, under the direction of his/her/their Supervising Physician and the Practice Administrator. The nurse practitioner will be responsible for providing substance abuse treatment, PrEP, HCV treatment, and gender affirming care. Candidates fluent in Spanish/English are encouraged to apply!
Qualifications: The nurse practitioner will be a licensed clinician with preferably three or more years post training experience providing primary care. Experience in providing medical care to underserved populations is required and experience in HCV, substance abuse treatment and PrEP care are preferred. The nurse practitioner will maintain up to date licensure in the state of Pennsylvania, and continuing education credits at all times. CPR certification is required. The nurse practitioner needs to have highly effective oral and written communication skills and have the ability to relate to and work with people of all ages, social and ethnic backgrounds. The nurse practitioner must have the ability to work independently and as part of a team and inspire confidence and trust in patients and staff and have the ability to understand patient needs as consumers of health care and focus service around those needs. Additionally, the ability and willingness to obtain hospital privileges is required.
Responsibilities:
Primary Care: Supervise and coordinate primary care for patients at the Philadelphia FIGHT Community Health Centers. Participate in both acute care and chronic care visits. Assure that all care is delivered in a manner that is compliant with all applicable laws and regulations, the requirements of payers, and consistent with all policies that affect Philadelphia FIGHT as an FQHC. The nurse practitioner will remain knowledgeable of applicable standards of care and treatment guidelines and will maintain continuing education credits as required by his, her or their professional licensure, to remain up to date on all such standards and guidelines. They must have the ability to understand patient needs as consumers of health care and focus service around those needs.
Substance Abuse Treatment: The nurse practitioner will be required to prescribe buprenorphine when appropriate. The nurse practitioner will also work with a Harm Reduction Behavioral Health Consultant on program development for a comprehensive Medication Assisted Treatment Program.
Clinic Sessions: Establish eight regular clinic sessions of four hours' time per week, at times to be determined by the mutual convenience of Physician and the Medical Director. Participate in after-hours call as a member of the clinic team. This provider is expected to take at-home, after hours calls for the clinic's 24-hour emergency line on a rotating basis - approximately 4 times annually (or every 3 months). This is an exempt position.
Quality Assurance: Participate in weekly case conference meeting, participate in ongoing peer review and other oversight protocols as necessary to assure that U.S. Public Health Service, DHHS, BPHC, HAB or other relevant guidelines are adhered to in the health center and that research conducted at the health centers by nurse practitioner is in conformance with standards of Good Clinical Practice. The nurse practitioner is also required to actively participate and be a clinical leader in quality improvement activities within the health center.
Confidentiality: They must maintain confidentiality of all patients, clients and research subjects of the FQHC in accordance with HIPAA.
Physical and Mental Demands: Normal health center/office/community environment; see, hear, talk, walk, sit and or stand for long periods of time, reach, stoop, bend, lift up to 25 lb.; repetitive hand movement; use and view PC. Read and write; verbal and written communication, basic math skills, analyze, prioritize, meet deadlines, and maintain high standard of confidentiality. Use sound judgment. Move throughout the health centers and community.
Work Environment: Generally, works in a high-volume, clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting. May also be required to provide coverage at all Philadelphia FIGHT Community Health Centers.
