Overview
Gifford has an opportunity for a Post-Acute Care Clinic Nurse Practitioner to join our well established, successful Primary Care Division. This position provides care to patients transitioning from an inpatient or ED stay back to their primary care clinician. Discharge from an inpatient or ED stay can be confusing for patients and their families and this position helps provide review, education and direction to patients to ensure their understanding of “next steps” in their care plan. The Clinician is expected to speak with patients about their illness, educating their family members, and partnering with other Clinicians to provide high quality, evidence-based care.

Duties and Responsibilities:
· Providing care to patients with recently discharged from a hospital stay or ED visit
· Educating patients and their families about their condition and what they should expect from the treatment plan
· Performing examinations and diagnostic test s to assess the patient’s condition
· Communicating with their Primary Care Clinician to determine the best care plan for every patient
· Providing acute care to patients as needed
· Keeping records that track patient progress
Requirements
· VT Licensure
· Ability to work with a high degree of accuracy and compassion in a fast-paced setting
