About the Job
Skilled Nursing Facility located in Riverdale has an immediate need for a Nurse Practitioner.
Summary:
Position is responsible for providing primary medical care and care management/coordination to a panel of assigned members residing in the contracted Skilled Nursing Facility.
S/he will collaborate with the member's PCP and the nursing home Medical Director.
S/he will conduct regular and acute visits / assessments in accordance with the Medical Staff by-laws of the Skilled Nurse Facility (SNF) and current Federal and State Health Code.
S/he will work with the nursing home ICT (Interdisciplinary Care Team) members.
Type: HC: Full-Time/ Perm
Schedule: Monday through Friday: 09:00am-05:00pm
Location: Riverdale
Requirements:
Board Certification from AANP or ANCC
Must have a minimum of two years full time experience as a Nurse Practitioner, preferably in clinical geriatric care
Current DEA, BCLS Certifications
Ability to work independently, develop a differential diagnosis and initiate treatmen
Ideal candidate must possess a graduate degree of a Nurse Practitioner program

Salary is Min. $120,000 - Max. $140,000
