Job Description:
This role involves providing primary healthcare to seniors, allowing them to live in their communities and homes. By working under the Medical Director's direction, the Nurse Practitioner collaborates with a team of physicians and interdisciplinary teams to offer comprehensive medical services. This includes assessments, care planning, and continuous medical support to enhance participants' quality of life.

Required Qualifications:
* Master's degree from an accredited nursing program.
* Certified Nurse Practitioner with Prescriptive Authority.
* Valid California NP State License.
* A minimum of two years' experience with the senior population.
* In-depth knowledge of chronic care and geriatric best practices.
* Clean DMV record and valid state driver's license.
* Recent vehicle insurance copy.

Preferred Qualifications:
* Prior experience in care coordination and policy improvement is an advantage.

Responsibilities:
* Conduct comprehensive, initial, semi-annual, and unscheduled assessments of participants.
* Collaborate with physicians and interdisciplinary teams to develop and implement care plans.
* Participate in discharge planning and maintain participant care in various settings, including nursing homes.
* Review data on outcomes and utilization to guide best practices and policy enhancements.
* Engage in rotating night and weekend calls.

Experience:
* At least 2+ years as a Nurse Practitioner, preferably in geriatric care or related field.
