**Description** Explore this unique full-time Nurse Practitioner opportunity to join the PeaceHealth St. John Medical Center Wound & Ostomy Care Clinic. This department is co-located with General Surgery, delivering an important service to our patients and community! PeaceHealth is passionate about growing this service for the community. The ideal Nurse Practitioner candidate will have both **Wound Care** post graduate experience and hold a certification in Wound Care & Ostomy or willingness to obtain within six months of hire. Ostomy experience preferred but not required. St. John Medical Center is 45 miles north of Portland and 120 miles south of Seattle. With 40- and 20-minute appointments. MA, RN, WOCN support using EPIC EMR. + Physician support onsite and virtual support with PeaceHealth Vancouver Wound Care & Healing Center. + Collegial, mission-driven, and collaborative culture. + PeaceHealth is the only provider of this service in Cowlitz County. + Responsible for clinic day to day operations, ordering DME/Wound supplies, and management of referrals. **Essential Functions:** + Debridement of wounds by sharp, enzymatic, autolytic or surgical methods able to be safely completed in the place of patient visit and all associated procedures to complete the debridement + Application and management of topical wound care and ostomy associated wound management products + Occasional peg tube dressing and wound care + Direction and recommendation of associated services to improve wound healing outcomes + Interpretation of diagnostic tests and recommendation of appropriate follow up + Responsible for ordering and providing recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice + Diabetic Foot Ulcer risk assessment for Members with Diabetes, as per the ADA guidelines Education of patients as to the topical and systemic management of their wounds + Education of the Primary Care Physician (PCP) or referring Physician on current wound management practices as well as the patient and their family + Provision of regular updates to the PCP or referring Physician on the status of their patients systemic conditions associated with wound care + Collaborates with staff to identify patients needs and to help develop plans to meet these needs + Proven ability to teach/train and advanced nursing knowledge **Hospital Highlights:** + PeaceHealth St. John Medical Center (https://www.peacehealth.org/sites/default/files/stjohnataglancefy20.pdf) is a 346-bed hospital with Level III trauma center with over 250 active medical providers with most specialty services provided locally to more than 180,000 patients per year + The service area for this community is over 200k **Experiencing Longview:** Recently named one of Forbes Magazine's prettiest American cities and a Tree City USA, is Longview Washington, an ideal location on the Columbia River and next to beautiful forests is just 40 miles north of Portland, Oregon, 2 hours south of Seattle, and a short drive to the beautiful Pacific coastline and year-round adventures on Mt Hood, Mt Rainier, Mt St Helens, and Mt Adams.
