Cuyahoga Falls Rehabilitation and Nursing Center is seeking a bright, compassionate, and professional Unit Manager (UM) to work in coordination with the Director of Nursing and Assistant Director of Nursing to support the nursing department functions of our rehabilitation and skilled nursing facility. Job Summary and Responsibilities: As the Nurse Unit Manager at Cuyahoga Falls Rehabilitation and Nursing Center, you will play a critical role coordinating and managing nursing units, ensuring high-quality patient care, and collaborating with the Director of Nursing to maintain regulatory compliance.
Leadership and Supervision: Provide leadership and supervision to nursing staff within assigned units. Foster a positive and collaborative work environment. Conduct regular team meetings to discuss patient care, challenges, and improvements. Assist with Infection Prevention, Risk Management, Staff Development, Scheduling, Central Supply, and Medical Records, as needed and directed by DON/ADON/LNHA.
Patient Care Management and Care Planning: Oversee the delivery of patient care services and ensure adherence to established standards. Collaborate with healthcare professionals to create and implement care plans. Monitor and evaluate patient outcomes, making adjustments as necessary. Attend Care Conference meetings on a regular basis with the interdisciplinary team, residents, and their respective families (upon admission, quarterly, upon discharge, and as needed.)
Staff Development: Participate in the recruitment, orientation, and ongoing training of nursing staff. Monitor and assess competencies of nursing staff on a regular basis. Conduct performance evaluations and provide constructive feedback. Identify opportunities for professional development and growth.
Quality Assurance: Implement and monitor quality assurance programs to maintain high standards of care. Address and resolve any issues related to patient care or staff performance. Evaluate areas for improvement as it relates to quality measures in resident care. Ensure compliance with state and federal regulations.
Communication and Collaboration: Collaborate with the Director of Nursing to implement policies and procedures. Communicate with physicians, nurse practitioners, and other vendors as needed. Foster effective communication between nursing staff, interdisciplinary teams, and management. Offer support and solutions to staff and IDT team when working through resident incidents. Act as a liaison between nursing staff and upper management. Assist in reporting and communicating of residents' condition changes to respective parties.
Documentation and Reporting: Maintain accurate records of resident care and ensure subsequent documentation compliance of nursing staff. Audit resident records and charts upon admission and on a regular basis to ensure accurate and up to date information. Ensure up to date and complete documentation for respective units from nurses and aides (including UDA’s, ADL tasks, MAR’s and TAR’s.) Provide regular reports to the facility administration when requested.
REQUIREMENTS OF UM:
- As the Unit Manager, you must possess strong interpersonal communication skills and the ability to deal effectively with patients, families, physicians, community resources, and the general public.
- Associate degree in Nursing or higher from an accredited school of nursing.
- Must have a valid OH RN or LPN license
- Minimum 1 years’ experience as a Unit Manager/Nursing Supervisor in Long Term Care experience may be considered
