Job Summary
The Home Health Coordinator maintains ongoing contact with health care organizations and the medical community to facilitate home health services for patients and participates with ongoing needs until the patient has been discharged. The Coordinator may attend physician office visits with the patient to help communicate any changes or feedback for patient care to the field staff. The Home Health Coordinator is also a resource for field clinicians as well as a liaison for physician communication for patient reports and orders for the plan of care.
- CoxHealth at Home, Home Health utilizes Coordinators for diverse Specialty Programs that include Wounds, Oncology, Orthopedics, Cardiology, Neurology, Pulmonology, and Diabetes.
Job Requirements
- Education
- Required: Graduate of an approved nursing program
- Experience
- Required: Minimum one (1) year experience as professional nurse within the last three (3) years.
- Preferred: Six (6) months home health experience.
- Skills
- Knowledge of Medicare and Medicaid regulations desirable.
- Licensure/Certification/Registration
- Required: MO RN License
- Required: BLS must be obtained within 90 days
- Required: Active Driver's License
