JOB SUMMARY The Nutritionists works cooperatively with all staff to ensure smooth and efficient clinic flow. Administers the nutrition related components of the WIC Program, following the policies and procedures of the NYS Department of Health WIC Program Manual. Determines participants' medical, nutritional and program eligibility.
ESSENTIAL FUNCTIONS:
- Provide Participant Centered Nutrition Services (PCNS) by phone or in person.
- Assist participants with WIC Health online nutrition education.
- Follows WIC Remote Work Guidelines.
- Enroll participants by completing a nutritional assessment, program eligibility and determine nutrition risk.
- Prescribe, tailor, authorize, issue, and reissue all food packages. Document nutrition services provided, including referrals and follow-up.
- Review and approve formula requests requiring medical documentation in consultation with the Health Care Provider.
- Identify the need for an individual care plan and refer to Qualified Nutritionist, as appropriate.
- Carry out high-risk care plans established by the Qualified Nutritionist and provide participant-centered counseling for high-risk participants. Work in conjunction with the Qualified Nutritionist to ensure continuity of care and to decide when the high-risk care plan can be closed.
- When applicable complete a breastfeeding assessment and provide participant centered counseling that helps a mother initiate breastfeeding, establish a milk supply, exclusively breastfeed and continue breastfeeding. Ensure participant is assigned to a Peer Counselor
- Address common breastfeeding concerns, issue breast pump and yield as needed to a Designated Breastfeeding Expert.
- Provide and document participant-centered nutrition education/counseling (individual/family sessions and facilitated group discussions) for participants.
- Track participant's progress, and document outcomes.
- Complete/attend Catholic Charities, WIC and DOH trainings.
- Complete required continuing education units as required by position.
- Develop, conduct and evaluate nutrition training for LA staff in nutrition education topics.
- Provide guidance to Nutrition Assistant staff in collecting nutrition assessment data.
- Assist and oversee all duties of a Nutrition Assistant.
- Orient, train, and mentor Nutrition Assistant staff.
- Attends staff meetings, Catholic Charities, NYWIC and NYSDOH trainings and achieves proficiency in NYWIC computer system.
- Comply with the Agency's Compliance Program, Code of Ethics, laws, regulations, and ethical standards applicable to your position in the WIC program and Catholic Charities.
- Travels to all sites, transports laptops, computers and other equipment to sites.
- May have to lift up to 30lbs.
- Performs other duties as assigned.
ACCOUNTABILITIES: WIC Program Coordinator and Coordinator
EDUCATION:
- RN with bachelor's degree
- Bachelor's degree in an appropriate health-related field with at least 6 hours' nutrition.
EXPERIENCE:
- Basic computer skills
Must have reliable transportation with the ability to travel to other WIC sites and transport equipment (sometime in the middle of workday) such as laptops, printers, breast pumps, medical equipment, and nutrition materials. Must provide internet if working remote.
