Shift Information:
This position is Full-Time position working Monday-Friday 8am-5pm.
Currently offering a $6,000 sign on bonus to qualifying full time Occupational Therapists.

What you will do:
The Home Care Occupational Therapist is a professional who coordinates and directs the home care patient’s care in their home environment (private residence, assisted living, etc.) based on individual needs. The Home Care Occupational Therapist is responsible for independent management of the Home Care patient population requiring use of advanced assessment, teaching, and decision-making skills. The Home Care Occupational Therapist cares for a caseload of home care patients by evaluating the patient for appropriateness of home care needs and developing the plan of care in collaboration with the provider, caregivers, and Home Care IDG members. The Home Care Occupational Therapist provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Home Care Occupational Therapist provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines, and best practice. The Home Care Occupational Therapist documents in the patient’s medical record in a timely manner according to agency policy. The Home Care Occupational Therapist attends and actively participates in Case Conference and team meetings. The Home Care Occupational Therapist performs on-call responsibilities. The Home Care Occupational Therapist supervises and delegates appropriate functions to the Certified Occupational Therapist Assistants.
Required Education:
Bachelor’s Degree in Occupational Therapy

Required Skills:
Therapy skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.

Required Licensure /Registration /Certification:
Current Texas license as an Occupational Therapist; CPR; Current driver’s license and auto insurance. Must have reliable transportation

Advanced skills (or) certifications in specialty areas, such as NDT, geriatric, orthopedics, lymphedema etc.

Work/educational experience in the healthcare field
