General Summary: The Occupational Therapist at OSF Home Care is responsible for visiting patients in their home to perform therapeutic evaluations, exercises and treatments to restore function and prevent disability following illness or injury as ordered by a physician and in accordance with State Practice Act and Home Care rules and regulations.Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients serves. This requires that the clinician demonstrates knowledge of the principles of growth and development, as well as physical, emotional, and psychosocial needs of the patient population served.Duties and Responsibilities:Evaluates patients' physical and functional status including musculoskeletal system, neurological system and functional mobility skills in activities of daily living and includes gathering information from the medical record, patient, and family/care giver.Analyzes evaluation results and sets appropriate short and long term goals to support/develop the patients’ plan of care.Identify, administers, or monitors treatment. Treatments may include: modalities, therapeutic exercise, transfers, functional mobility training, ADL/IADL training, adaptive equipment training, and home safety.Determines appropriate assistive devices such as dressing aides, transfer aides, orthotics, prosthetics, etc. which are needed for functional mobility, safety, energy conservation, and ADL/IADL activities; including patient and family teaching as needed.Reassesses patients’ response to treatment and adjusts program accordingly; includes ongoing communication with the physician, occupational therapy assistants, patient and/or family and other health care disciplines.Recommends other disciplines, programs, and equipment/ supplies as needed by the patient and/or family
