Occupational Therapist:

Occupational Therapist , PRN for weekdays and weekends 

Polaris Health Care and Rehabilitation Center is looking for an experienced Occupational Therapist to join our rehabilitation team and provide therapy services to patients in our skilled nursing facility. The ideal candidate will be passionate about their patients and committed to helping them recover their independence.  The Occupational Therapist will partner with patients in our facility to create a functional plan of care tailored to the patients’ goals. The right candidate is patient, compassionate and professional who encourages and inspires their patients to work toward greater independence and recovery.

Responsibilities for Physical Therapist:

• Develops and implements an individualized treatment plan and tailored to the patient abilities to restore function and independence
• Monitor patients' progress and adjust treatments accordingly.
• Maintain positive interactions with patients, families and caregivers while providing instruction and education in home programs, basic living skills, and use of adaptive equipment.
• Provide supervision for Occupational Therapist Assistants, students and support personnel in accordance with state license requirements and professional standards

Qualifications for Occupational Therapist:

• Experience providing occupational therapy to patients in a skilled nursing facility
• Current Occupational Therapy license for state of Delaware
• Current CPR certificate
• Requires good organizational and time management skills
• Requires good verbal and written communication skills
