Our Award Winning Client is seeking a highly qualified Occupational Therapist to join their team, responsible for providing in-home Occupational Therapy services to Agency patients, as prescribed by the attending physician. The role also involves offering consultation to Agency staff and physicians.
Key Responsibilities
- Perform Occupational Therapy assessments to evaluate patient functioning levels, develop treatment programs, and set short and long-term goals aimed at restoring independence and physical/cognitive functioning.
- Provide guidance and instruction to patients/family members on therapeutic, creative, and self-care procedures, and explain adaptive and progressive approaches for implementation.
- Instruct patients/family members in the use of orthotic or prosthetic equipment and may design and fabricate customized adaptive/assistive devices.
- Conduct functional and cognitive re-evaluations to revise treatment plans as necessary.
- Maintain clinical and progress notes, submitting paperwork in a timely manner according to Agency policy.
- Report periodically to physicians on patient treatment, response, and progress, assisting in treatment plan adjustments.
- Coordinate outpatient therapy services when home treatment is not feasible.
- Provide consultation to other home health team members regarding necessary equipment, techniques, and activities.
- Participate in staff development, in-service attendance, and Agency development activities.
- Attend staff meetings, in-services, and case conferences as required.
- Other duties as assigned by the supervisor.
Qualifications
- Required
- Bachelor’s degree or higher
- Occupational Medicine (1+ years)
- Social Work (2+ years)
- Valid Occupational Therapist, Registered (OTR) in Pennsylvania
- Valid Regular Driver's License (DL)
- ANY of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Pediatric Advanced Life Support (AHA PALS)
- Preferred
- Please provide your OT license number.
Schedule and Shift Details
- M-F, 8am-5pm, no weekends, holidays, or on-call.

Compensation
This is a Pay-Per-Visit (PPV) position. Rates vary based on the type of service and level of experience.
