We are seeking a licensed Occupational Therapist to join our team at a skilled nursing facility. As the Occupational Therapist, you will be responsible for evaluating and treating residents with physical, cognitive, and psychosocial impairments to help them achieve their highest level of independence and quality of life.
Responsibilities:
- Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments
- Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
- Provide skilled occupational therapy interventions, including therapeutic exercise, activities of daily living training, cognitive retraining, and environmental modifications
- Monitor and document resident progress and adjust treatment plans as necessary
- Communicate with interdisciplinary team members, including physicians, nurses, physical therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents
- Educate residents and family members on occupational therapy goals, treatment plans, and home exercise programs
- Maintain accurate and timely documentation of occupational therapy services provided
- Participate in team meetings, in-service education, and quality improvement initiatives
- Maintain current knowledge of occupational therapy practice and relevant regulations
Qualifications:
- Current license to practice as an Occupational Therapist in the state of California
- Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
- Knowledge of current occupational therapy practice standards and regulations
- Proficient in using electronic medical records and other healthcare software
