Job Description:
Summary/Objective
As part of a vibrant team, participate in providing client related therapy services, including but not limited to, activities which assist clients in restoring or improving independent function relative to activities of daily living within the scope of practice to SummitWest Care clients.
Essential Functions
1. Performs initial and ongoing client assessments and applies appropriate goals and interventions to resolve identified problems.
2. Provides Occupational therapy services to client according to physician orders and regulations.
3. Assures physicians are notified in a timely manner of changes in client status and physician s orders are obtained as needed for changes to the plan of care.
4. Identifies and obtains/makes appropriate modifications to equipment/environment to meet client needs.
5. Provides teaching and instruction to client/caregiver to promote physical and emotional health.
6. Attends required staff meetings and case conferences.
7. Contributes to the coordination of services through effective and timely communication with other members of the health care team.
8. Maintains the caseload and agency productivity standards.
9. Responds to referrals to Occupational Therapy in a timely manner and documents care according to the agency standards.
10. Maintains clients self-esteem, privacy, and confidentiality of personal information.
11. Assures client rights are protected and that all clients/caregivers are made aware of the client s rights and responsibilities.
12. Initiates emergency procedures as appropriate.
13. Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.
14. Performs other duties within scope of practice, as assigned by Sr. Director of Clinical Operations
15. May require extended staff schedules.
16. Other duties as assigned.

1. Communicate clear, concise, relevant, non-judgmental information.
2. Maintain open lines of communication with all staff.
3. Utilizes effective problem-solving/conflict resolution skills.
4. Promotes supportive group process.

1. Demonstrates knowledge of applicable Occupational Therapy standards in accordance with Medicare Conditions of Participation.
2. Accepts responsibility for personal and professional accountability for own practice.
3. Complies with and enforces all agency policies and procedures.
4. Demonstrates effective priority setting and time management skills.
5. Performs duties independently, without supervision.
6. Presents a positive image of the agency.
7. Demonstrates an attitude of caring when dealing with clients, families, and staff.
8. Complies with ethical conduct and displays competency in performing assessments and treatment techniques according to professional standards.
9. Complies with timely, accurate, and appropriate documentation of care and communications pertaining to patient care.
