Position Summary:
We are seeking a compassionate and driven Community Resource Navigator to join our team as a Community Health Worker. In this role, you will be pivotal in connecting individuals and families with essential resources and services to improve their overall well-being. You will utilize an innovative Community Information Exchange (CIE) platform to enhance service delivery and streamline access to health and social services. Your role will involve collaborating with local organizations, healthcare providers, and community leaders to address barriers to care and promote health equity.
This position is partially funded through local and state grants. Continuation of the position as-is will be dependent upon sufficient organizational funding.Job responsibilities are subject to change.
Roles and Responsibility:
- Resource Coordination: Assess clients' needs and assist them in navigating and accessing various health and social services, including medical care, financial assistance, housing, transportation, food and utility support. Utilize the Community Information Exchange (CIE) platform to track and manage client referrals and service coordination.
- Education and Advocacy: Educate community members about available resources, health practices, and preventive measures. Advocate for clients' needs and rights to ensure they receive appropriate services.
- Case Management: Provide personalized support and follow-up for clients, including goal setting, service coordination, and monitoring progress. Maintain accurate and confidential client records using the CIE platform to ensure seamless communication and continuity of care.
- Outreach and Networking: Conduct outreach activities to engage underserved populations and raise awareness about available resources and services. Collaborate with local agencies, health professionals, and community groups to enhance service delivery.
- Data Collection and Reporting: Collect and analyze data on community needs and service utilization. Utilize the CIE platform to generate reports and contribute to evaluations to assess the effectiveness of programs and services.
- Training and Development: Participate in ongoing training and professional development to stay informed about best practices, emerging trends, and changes in the health and social service landscape. Gain proficiency in using the CIE platform and participate in related training.
Education, Experience and Qualifications:
·High School Diploma or GED required
·Associates Degree in Social Work, Public Health or related field preferred
·Minimum of 2 years of experience in community health, social services or related field. Experience in case management or resource navigation is highly desirable.
·Certification as a Community Health Worker (CHW) preferred, or willingness to obtain certification
·Strong interpersonal and communication skills, with the ability to build rapport and establish trust with diverse populations
·Excellent organizational and problem-solving abilities
·Proficiency in using Microsoft Office Suite and other relevant software
·Knowledge of local community resources and health services.
·Ability to work independently and as part of a team
·Understanding of and commitment to the mission, vision, and goals of United Way
·Must have a valid driver’s license and reliable transportation. Bilingual skills are a plus.
Working Conditions:
·Primarily field-based with some office work. May require occasional evening and weekend hours.
·Ability to travel within the community, including visits to community organizations. Occasional lifting and carrying of materials.
