Duties and Responsibilities

Evaluate patients' physical, cognitive, and emotional status to develop personalized treatment plans.
Perform assessments of daily living activities such as dressing, grooming, and mobility.
Design individualized rehabilitation programs aimed at improving function, reducing pain, and preventing further injury.
Collaborate with a multidisciplinary team (physicians, nurses, physical therapists) to align therapy goals with medical care.
Provide therapeutic exercises, mobility training, and manual therapy to improve strength, coordination, and range of motion.
Use adaptive equipment or modifications to enhance patient independence in performing daily tasks.
Work with the discharge team to determine whether patients can return home, need rehabilitation, or require other support services.
Recommend home modifications or equipment (e.g., grab bars, walkers) for safety and functionality.
