Position Summary:
Join our dynamic care team composed of Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, Referral Coordinators, and more. This comprehensive support system enables our primary care physicians to focus on quality care, seeing fewer patients while dedicating more time to each individual.
Primary Responsibilities:
- Evaluate and treat patients in alignment with established standards of care.
- Coordinate patient services, including specialty referrals, hospital and skilled nursing facility (SNF) transitions, durable medical equipment, and home health care.
- Actively engage as a key medical resource within the care team during daily huddles.
- Collaborate with the Lead Physician and Center Administrator to foster a cooperative practice environment through professionalism and approachability.
- Complete all medical record documentation daily, partnering with a quality-based coder to enhance comprehensive documentation.
- Adhere to policies and protocols set forth by Clinical Leadership.
- Participate in meetings with the Lead Physician to discuss quality of care, review outcome data, and address medical record concerns.
- Maintain a strong focus on direct patient care, balancing patient-facing responsibilities with necessary administrative tasks.
Required Qualifications:
- Graduate of an accredited MD or DO program.
- Current and unrestricted medical license, or willingness to obtain a license in the state of practice; must also be eligible for licenses in other states as needed.
- Board Certified or eligible for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Exceptional interpersonal skills and effective communication with colleagues and patients.
- Commitment to a team-based care model.
- Willingness and capability to adapt to a value-based care environment.
- Outstanding patient and customer service skills.
- Basic proficiency in computer skills, including email and electronic medical records (EMR).
Preferred Qualifications:
- Familiarity with HEDIS/STARS measures.
- Understanding of CAHPS and HOS.
- Experience serving medically underserved and culturally diverse communities.
- Proficient in working with electronic health records.
- Experience with Medicare Advantage Global Risk Models.
