Outpatient Registered Dietician/Coordinator of Diabetes Self Mngmt Program - Cheshire Medical Center

The Outpatient Registered Dietitian is responsible for the education and counseling of our patients at our organization. Responsibilities include all outpatient services and community education programs; along with being responsible for the management, implementation, maintenance and evaluation of the Diabetes Self-Management Program. This role also functions as part of the multidisciplinary health care team at Cheshire Medical Center. Participates as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes and other chronic diseases. Provide individual Medical Nutrition Therapy/counseling and education to patients at Dartmouth-Hitchcock Keene. Prepare and update Diabetes program policies and procedures. Individually assess and provides education to Diabetes program participants including relevant medical history, cultural influences, health beliefs and attitudes, diabetes knowledge, self-management skills and behaviors, readiness to learn, cognitive ability, physical limitations, family support and financial status. Counsel/educate individual patients appropriately on diabetes care. Requirements: Bachelor degree in Nutrition. A minimum of 3-5 years experience of individual nutrition counseling. Experienced with individual counseling and presenting community education programs. Academic preparation and or experience in program management. Commission on Dietetics Registration as RD and NH licensure required upon hire. Certified Diabetes Educator by the National Board for Diabetes Educators preferred.
