The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services.

The Mission Neighborhood Resource Center (MNRC) was founded in 2002 as a community response to the neighborhood's growing homeless population, the Mission Neighborhood Resource Center advocates for housing and resources to enhance the physical, social, emotional, and economic health of homeless residents in the 16th street corridor and surrounding Mission neighborhood. MNRC employs a service model based on harm-reduction principles and practices that focus on community building, basic human needs, and integrated health services for a diverse population as they move toward greater stability and well-being.
We are currently looking for qualified candidates for our Outreach & Enrollment Specialist role. Please note: this role requires an applicant to be bilingual (English/Spanish). This position reports to the Program Director and is a Full Time, Non-Exempt role with a starting hourly range of $23.52 to $25.26 with full benefits.
Primary Objective:
Under supervision, the Outreach and Enrollment Specialist conducts focused outreach to educate and assist patients and community members navigate the enrollment process for health insurance coverage (Medi-Cal and/or Covered CA) and health care assistance programs (Healthy San Francisco) for the uninsured. The Specialist works to increase health care access and coordination; insurance coverage and continuity by effectively communicating with assigned patients, clinical and health education departments, and health insurance/health care assistance programs. Assists patients with communications to social services, health options and health center processes.
Essential Functions/Responsibilities
Direct Patient & Client Services
- With a high level of customer service assists Mission Neighborhood Health Center (MNHC) patients and community members with eligibility determination and insurance enrollment (Medi-Cal and Covered California) through the electronic patient portal California Healthcare Eligibility, Enrollment & Retention System (CalHEERS) website or through My Benefits CalWin. For adult MNHC patients not eligible for insurance enrollment, assists them in enrolling them into Healthy San Francisco through One-e-App.
- Contacts Managed Care Health Plan members who are assigned to MNHC to register and/or schedule medical appointments.
- Assists patients navigate through MNHC and the healthcare system by providing the patient with direct referrals, and warm hand offs to appropriate department staff, finding relevant social services resources and being a direct link for the patient and resource specialist.
- Conduct educational activities at identified venues and organizations in MNHC’s service area among community members to raise awareness about insurance coverage options.
- Educates community members and organizations of the services offered by the clinics of the Health Center.
- Must have a flexible schedule for community events and outreach opportunities after business hours i.e., evenings and weekends.
- Expand on established relationships with outside organizations; develop new referral relationships with outside social services organizations.
- Assists in maintaining the database to identify and track the number of outreach contacts, educational sessions provided, and the number of applications submitted.
- Submits monthly statistical data to Outreach and Enrollment Supervisor and other report owners in a timely manner.
- Coordinates activities with Patient Services staff from MNHC, San Francisco Community Clinic Consortium (SFCCC) clinics, and other service and enrollment organizations throughout the City.
- Participates in periodic program evaluation activities (e.g. effectiveness of Patient In-reach & Community Outreach Work Plan)
- Participates in regular meetings with other Outreach & Enrollment staff to share the results of activities and best practices.
- Performs other duties as assigned.
Qualifications
Education
- Associates of Arts degree or equivalent.
- Must have Certificate as a Certified Application Counselor (CAC) for Covered California, Medi-Cal and Healthy San Francisco or successfully complete training and achieve certification within 6 months of hire
- Must be eligible to re-certify annually as a Certified Application Counselor (CAC) for Covered California, Medi-Cal and Healthy San Francisco.
- Working knowledge of Latino cultures preferred.
- Experience working with underserved and vulnerable populations.
- Ability to work collaboratively and communicate effectively (verbal & written) with others, including patients and families, staff from different departments and the public community at large.
- Ability to take initiative and exercise independent judgement to support program development and success.
- Experience conducting outreach and in-reach and in developing strategies to overcome barriers to health care access and care preferred.
- Excellent communication and organization skills, attention to detail
- Demonstrated problem-solving skills are required.
- Uphold ethical standards, practices, department’s policies/procedures and organization’s policies/procedures.
- Data management proficiency, including Microsoft Office software and web-based data entry
- Work knowledge of general office equipment
- Must adhere to all MNHC policies and procedures on confidentiality and HIPAA.
- Bilingual Spanish/English required
