Job Overview - Outreach Specialist:
Do you have a knack for connecting with people and a passion for community outreach? Join our dedicated team as an Outreach Specialist in Staten Island, NY, where you'll play a crucial role in identifying and enrolling new clients for our Health Homes program.

Location: Staten Island, NY
Compensation: $22/hour
Schedule: Monday to Friday, 9:00 a.m. - 5:00 p.m.

Responsibilities as the Outreach Specialist:

• Identify clients for outreach in coordination with the Outreach Supervisor.
• Conduct home visits, phone calls, and mail letters to enroll eligible clients.
• Document progress notes and screening tools for clients.
• Drive agency vehicle to provide outreach services.
• Maintain accurate documentation in the program's database.

Qualifications for the Outreach Specialist:

• High School Diploma or equivalent.
• Experience with the homeless or HIV population preferred.
• Bilingual in Spanish/English is a plus.
• Clean, valid driver's license with the ability to drive a passenger van required.
• Exceptional communication skills, both oral and written.
