MINIMUM QUALIFICATIONS: High School Diploma or Equivalent. Assist and support people with disabilities. Applicants must be physically fit. The employee must have the ability to lift / move at least 50 pounds.
SERVICE PLANNING AND IMPLEMENTATION: Participate as a member of the individual's interdisciplinary team. Conduct assessments and evaluations. Write training programs which identify short term and long term goals, objectives and/or needs. Responsible for the implementation of both informal and formal training and record documentation and outcomes of training.
MEDICAL SERVICES: Responsible for medication administration as assigned, adhering to all agency policy and procedures.
GENERAL RESPONSIBILITIES: Responsible for daily attendance, supervision and transportation of individuals served. Provide input into the individual's physical, mental, and financial well-being to the individual's case manager, supervisor, and services coordinator in writing or verbally of the overall quality of the individual's services being provided. Provide services to include assistance in feeding, bathing, dressing, toileting, and transporting. Assist in planning, preparing and serving meals in accordance with the dietitians menu's and specific individual dietary and nutritional needs. Maintain a clean, safe area for the individuals served. Maintain all training supplies, equipment and work/living areas in safe, clean working order. Perform all other duties as deemed necessary.
