Overview

We are looking for a Part-Time (3 days/week 7.5/hr shifts) Registered Nurse for a Home Health and Hospice Intake Coordinator position. The Home Health and Hospice Intake Coordinator will mostly work in a hospital setting but will be required to go to the main office located in Northport from time to time. The RN Intake Coordinator manages and facilitates the transition of patients from the hospital to their home. The RN Intake Coordinator will act as a primary liaison for the agency between physicians, hospital discharge planners, social workers and related community health services referral sources. This position maintains timely and accurate records on all patient referrals, community contacts, and quality care information and reports same to Director of Intake and Service Development to be utilized in planning and scheduling quality improvement and follow-up. The pay and benefits are generous for this RN Intake Coordinator position and even at part-time hours, the RN would qualify for benefits! It is preferred that the RN have Home Health or Home Hospice experience, however we will consider Registered Nurses who have at least 1 year of recent (within the past 3 years) acute experience in a hospital setting. Requirements of the Home Health and Hospice Intake Coordinator Registered Nurse position: It is preferred that the RN have Home Health or Home Hospice experience, however we will consider Registered Nurses who have at least 1 year of recent (within the past 3 years) acute experience in a hospital setting. The registered nurse must be a registered nurse (RN).  The registered nurse must be licensed in the state.
