A DAY IN THE LIFE OF A PATH HOMELESS OUTREACH CASE MANAGER

In this part-time role, your day begins with a sense of purpose as you set out to connect with individuals facing homelessness and mental health challenges. As a PATH Homeless Outreach Case Manager, you are their guide, their advocate, and their source of support. QUALIFICATIONS

• Ability to work with individuals with severe and persistent mental disabilities
