As a Patient Care Coordinator, you’ll work closely with healthcare professionals, patients, and their families to coordinate personalized care plans. Your work will be essential in delivering timely, compassionate care and helping to build strong relationships with everyone involved.

Key Responsibilities:

• Collaborate with healthcare providers to manage and coordinate patient care plans.
• Assist in scheduling home visits, ensuring that patients receive the care they need, when they need it.
• Communicate clearly and effectively with healthcare teams, patients, and families to ensure all parties are informed.
• Maintain accurate records of patient care plans and any modifications to ensure seamless care coordination.
• Support patients and their families by educating them on available resources and services.

What We’re Looking For:

• A Patient Care Coordinator who is a team player with strong problem-solving skills.
• Excellent social and communication skills to build lasting relationships with patients and healthcare providers.
• Willingness to travel within the designated territory to visit various facilities and engage with healthcare staff.
• Proficiency in Microsoft Suite and basic computer skills.
• An eagerness to learn and grow in the healthcare field while taking on new challenges.

Daily Tasks Include:

• Engaging with office managers, physicians, and patients as the go-to Patient Care Coordinator.
• Documenting and promoting the company’s services while traveling to different facilities.
• Generating referrals and helping facilities meet patient care needs.
• Identifying patients who may need additional care and obtaining approvals to ensure proper treatment.
