Job Details
In this role you will navigate newly diagnosed cancer patients from the surrounding communities to receive timely, patient-centered care. The PN will incorporate 1) screening for a history of severe mental illness and risk factors for disruptions in cancer care at cancer diagnosis, 2) screening and monitoring for distress, and 3) streamlining referral/early consultation of social work, psychiatry, and palliative care. The PN will be a patient liaison with oncology and primary care team ensuring open lines of communication between the two teams.

Job Details
In this role you will navigate newly diagnosed cancer patients from the surrounding communities to receive timely, patient-centered care. The PN will incorporate 1) screening for a history of severe mental illness and risk factors for disruptions in cancer care at cancer diagnosis, 2) screening and monitoring for distress, and 3) streamlining referral/early consultation of social work, psychiatry, and palliative care. The PN will be a patient liaison with oncology and primary care team ensuring open lines of communication between the two teams.
Responsibilities
- Collaborate with patients, providers, and the Cancer Center team to set care goals and document them.
- Help patients navigate logistical, scheduling, or childcare barriers to attending appointments.
- Address insurance concerns and connect patients with financial resources.
- Assist in connecting patients to supportive care, social work, and palliative services.
- Educate patients on care plans, potential side effects, and clinical trials, connecting them with resources as needed.
- Provide culturally sensitive support to patients from diverse backgrounds.
- Communicate with providers via EMR notes, emails, calls, and case reviews.
- Refer patients to case management services for additional needs (e.g., hunger, domestic violence).
- Advocate, educate, and support patient access to community and hospital programs.
- Document interventions in the EMR.
- Build relationships with oncology schedulers and related services.
- Work with medical interpreters to support non-English-speaking patients.
- Prepare mid-year and end-of-year program activity reports.
Experience:
- At least (5) years of hospital nursing experience, outpatient preferred
- At least two (2) years of professional experience in an oncology or outpatient infusion setting required
- 2 years of previous chemotherapy experience required
- Previous patient navigation experience preferred
- Previous experience working with culturally diverse populations required.
- Possess strong working knowledge of Oncology Programs
- Strong understanding of community resources for oncology patients and assisting with referrals
- Demonstrated commitment to impacting the care of high-risk patients
- Knowledge of complex patient issues preferred
- Able to establish strong working relationships within the medical community required.
- Experience with specialty pharmacies
Licensure/Certification:
- Current RN license in good standing required.
- Certified Oncology Nurse Required (OCN) required or must become OCN credentialed within 18 months of hire date.
- BSN Preferred
- American Heart Association Healthcare provider course with Basic Life Support (BLS) certification required to be initially completed with the Staff Development Office within 30 calendar days of hire date.
