GENERAL RESPONSIBILITIES
This role works closely with Behavioral Health providers with a focus on improving access to health care services for TRHS patients in Canyon County. The successful candidate will be able to increase health and screening and use of healthcare services by transporting patients and through home visits.
MINIMUM QUALIFICATIONS
- Bachelor's degree in social work or related or 4 years equivalent experience
- 3 years' experience in a primary care setting
- Successful clearance of Department of Health and Welfare Criminal Background Check.
- Experience developing and teaching community education classes.
- Comprehensive and documented experience with case management for complex patients with barriers to care
- Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
- Comfortable with and successful at public speaking.
- Knowledge of community agencies and resources and ability to connect patients with services.
- Skill in use of personal computers and related software applications, including but not limited to: Microsoft Office Suite, EHR and web applications.
- Available for regular weekend and evening duties and multiple non-traditional work sites such as outreach events and home visits.
- Ability to work independently with minimal supervision and use sound judgment.
- Well-organized, creative, flexible, open-minded and able to manage several tasks simultaneously.
PREFERRED QUALIFICATIONS
- Medical Assistant background
- Two years' experience with Medicare and major insurance carrier regulations, procedures, and benefit plans.
- Public speaking and/or training experience
- Experience working within a mental health setting and with victims of sexual abuse, sexual offenders, domestic batterers.
- Bilingual in English and Spanish.
- Knowledge and experience with health promotion
- Experience working with underserved and/or aging patient populations.
- Experience using Electronic Health Record Systems (EPIC preferred), including scheduling.
