Rate Range: $50.86 - $71.22
Sign-on/Retention Bonus: $10,000 /Full Time Only
Job Summary: To enhance the patient's experience and ensure they receive the added support and guidance they may need when navigating a complex treatment journey. The Patient Navigator Registered Nurse is responsible for providing individualized support to patients during hospital stays or surgical visits. While helping the patients communicate with their healthcare providers within and outside of the organization. Ensuring accurate appointment times are coordinated and with the necessary resources available. The navigator will work with other care providers, vendors, and hospital personnel to ensure a seamless patient experience throughout the patient stay. Actively and consistently contributes to department operations and communications, behaves in a manner consistent with the mission, vision, and values of Pipeline Health, upholding standards of AIDET (Acknowledge, Introduce, Duration, Explanation, Thank you) patient communication.
Outpatient Procedures:
- Patient Navigator checks the schedule to coordinate PAT appointments and pre-op interviews.
- Conducts pre-op phone call with patient for nursing assessment and education.
- Ensures the patients are clear on admission statues ie… Inpatient stay or same day discharge.
- Patient Navigator communicates with the Unit Charge Nurse to inform patient is scheduled for surgery/procedure and coordinates direct admit bed availability.
Inpatient Procedures:
- Patient Navigator verifies the Primary Nurse has completed pre-op orders and starts the OR Readiness Check list (Paragon). (Utilization of the Anesthesia Pre-Procedure Guidelines) Reviews labs, consents, and diagnostic test. (Communicate Alerts to Anesthesia).
- Patient Navigator visits at bedside when necessary for confirmation of readiness.
- Patient Navigator communicates with the Charge Nurse (Supervisor may have primary report out) for patient’s readiness, report, and organizes transport.
- Patient Navigator notifies the OR Charge Nurse when the patient is brought to pre-op holding.
- Patient Navigator checks patient in verifying correct patient making sure information on wrist band matches information in chart (i.e. VID#, MRN, DOB), records, v/s, and full body assessment.
Experience:
- One (1) year experience in Perioperative Services.
- Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, strong organizational skills, ability to follow verbal instructions, and PC (computer) skills.
- A capacity to learn, synthesize, make critical judgments, work independently, place patients and families first, and collaborate with the team members who are recognized leaders within health care.
Education:
- Bachelor of Science in Nursing, required.
Licensure/Certifications:
- Current RN License issued and/or recognized by the state of California, required.
- Basic Life Support (BLS) from American Heart Association (AHA), required.
- Advanced Cardiovascular Life Support (ACLS) from American Heart Association (AHA), required.
- Pediatric Advanced Life Support (PALS) from American Heart Association (AHA), required.
- AB508 Workplace Violence Prevention (4 hour) due within 30 days.
- OR Management of Assaultive Behavior (MAB 1 or MAB 2), Crisis Prevention Intervention (CPI) or other certification covering AB508 and SB1299
