Software Guidance & Assistance, Inc., (SGA), is searching for a Patient Navigator Specialist for a Contract assignment with one of our premier Healthcare clients in San Francisco, CA .
Responsibilities :
Prepare patient clinic charts, abstract data from complex medical records and assist in compiling detailed treatment summaries; schedule patients for clinic appointments and act as patient and clinician liaison to ensure smooth clinic operations; educate patients/families about the clinic process and perform operational tasks; communicate daily with patients and other hospital subspecialty departments to coordinate patient care; maintain the H2M patient database; helps to coordinate insurance authorization for neuropsychologist appointments, prepare educational materials and maintains and updates departmental administrative manual; act as an intermediary between services and departments and maintaining continuum of care between inpatient to outpatient clinic ; manage intake packets ; create, clean, update, and manage databases and comprehensive datasets and reports; report patient progress to management; perform other duties as assigned

Required Skills:
- Candidates must have: experience running outpatient clinics where they support patients in clinic.
- Acquiring knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Knowledge of data collection, compilation, and analytical techniques.
- Skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. Working knowledge of the organization and how to get issues resolved.
- Solid interpersonal and customer service skills. Ability to be diplomatic and highly organized. Ability to remain calm under pressure and apply sound judgment.
- Ability to work independently in a fast-paced, demanding environment with minimal supervision. Ability to manage and oversee multiple tasks simultaneously, including high daily call volume.
- Ability to communicate effectively, both orally and in writing.
- Proficiency with Windows-based software including Microsoft Word, Excel, Outlook. Knowledge of computer systems and software used in functional area.
Preferred Skills:
- Previous clinical experience.
- Bilingual in Spanish.
