About the Job
Our growing healthcare client is looking for a Patient Access Representative on a contract to hire basis for their office in Oak Park.
In office position.
8:00am - 5:00pm scheduled Monday through Friday.
Responsibilities of the Patient Outreach Specialist:
- Respond promptly to referrals and inquiries from various channels, acting as the main contact for prospective patients and referral partners. Nurture leads by addressing inquiries, providing service information, and making external referrals when necessary, with the aim of converting leads into scheduled appointments.
- Handle phone calls, emails, and general administrative duties.
- Assist with follow-up communications after meetings with referral partners.
- Track and manage referrals from physicians, hospitals, and other healthcare providers.
- Maintain CRM and EHR systems with detailed records of interactions, sales activities, and contact information for patients and referral sources.
- Gather and compile data to generate reports on referral conversion rates and sources.
- Collect patient details, medical histories, billing, and insurance information, organizing them within the database or EHR system.
- Inform patients about office procedures, policies, and protocols.
- Collect feedback from patients and referral sources to guide process improvements.
- Collaborate with various departments to enhance patient access, reduce wait times, and improve the overall patient experience.
- Coordinate between therapists, case managers, and referral sources to ensure seamless continuity of care.
- Assist in preparing outreach materials for hospitals, referring physicians, or insurance providers.
- Support the coordination of promotional campaigns, outreach events, fairs, or educational seminars aimed at referral partners and community organizations, ensuring effective distribution of materials.
- Support the Sales and Marketing teams by participating in community events and relationship-building activities as needed.
- Provide a positive patient experience, maintaining excellent working relationships with patients, care teams, and staff, and demonstrating empathy, professionalism, and a patient-centered approach.
- Participate in TJC accreditation activities as required for the position.
- Comply with professional, ethical, and legal standards.
- Adhere to the company s philosophy of care, mission, values, and corporate culture.
- Ensure compliance with internal policies, agency regulations, and generally accepted practices.
- Work with mental health, TBI, and other patient populations as needed.
- Perform other duties as assigned.

- High School diploma required; Bachelor s degree in Business Administration, Marketing, Communications, or a related field preferred.
- 1+ years of experience in lead management, sales, or community outreach preferred.
- Excellent communication and interpersonal skills.
- Strong organizational abilities with the capacity to multitask and manage time effectively.
- Familiarity with CRM and EHR systems and proficiency in Microsoft Office Suite.
- Ability to work both independently and as part of a team.
