JOB SUMMARY:
The Physician/Patient Navigator shall focus on existing and new referral development, building strong partnerships with physicians and their staff to maintain and grow referrals from physicians and/or other providers for Houston Physicians Hospital services and programs. The Physician/Patient Navigator will also participate in various outreach activities to assist in growing specific outreach programs and navigate patients through the HPH system. The Physician/Patient Navigator will work with Clinical and Business teams in fulfilling this role and will be responsible for executing, tracking, and reporting weekly progress of the marketing plan and complete other duties as assigned.
QUALIFICATIONS:
- High School Diploma or equivalent required; Bachelor’s Degree preferred
- Previous experience in clinical and business environment in a healthcare setting
- 1-2 years marketing experience preferred in the health and/or rehabilitation industry.
- Excellent computer skills and thorough knowledge of EMR systems, Microsoft Office products and Smartsheet.
- Critical thinking skills necessary to plan, coordinate, and execute strategic initiatives
- Excellent customer service skills in person, over the phone and via e-communications
- Experience with performance and quality improvement activities
- Ability to work independently and under pressure; establish priorities and coordinate work activities effectively.
- Ability to maintain strictest confidentiality.
- Ability to communicate complex information in a well thought-out easily understood manner, both orally and in writing in a variety of settings and styles;

RESPONSIBILITIES:
- Patient Navigator

- MR Guided Focused Ultrasound Treatment Program
- Spine Solutions Center
- Vascular Solutions Center

- Others as assigned
- Program responsibilities include, but are not limited to:
- Manage new patient intake
- Manage new patient referrals into physicians’ private practices
- Coordinate care with various community care providers / stakeholders
- Coordinate logistics with Physician offices
- Track and trend patient data
- ROI data management
- Co-lead weekly team meetings with hospital staff, neurosurgeon office staff and Insightec staff
- Initial triage of patients
- Marketing
- Community
- Creating collateral pieces
- Patient and family navigation up to and including date of procedure
- Use teach-back method for patients’ home exercise program
- Coordination of care: pre-admission testing, insurance authorization, neurosurgeon, physician assistant, anesthesiology, preop, RAD, recovery and registration
- Track and trend all patient data; report clinical outliers to CQI quarterly meeting
- Post-procedure communication with referring physician
- Various outreach programs.
- TPA Program
- Manager Intake process for varies TPA programs (Transcarent, Carrum, Kempton, etc)
- Work closely with Marketing Team to:
- Manage new patient intake

- OutReach/Business Development:
- Relating to the Patient Navigator role, there are Outreach/Business Development activities:
- Work with Marketing Team to foster strong professional relationships and rapport with existing and future referring physicians and support staff.
- Develop growth for TPA strategies targeting new & existing TPA's
- Analyze existing and potential volume of referrals on a consistent basis.
- Develop and maintain accurate, current database of physician contacts.
- Prepare any reports regarding Business Development activity as requested
- Keep management informed by submitting activity and results reports
- Meetings – Including, but not limited to:
- Participate in sales, marketing and operations meetings as scheduled.
- Attend marketing and corporate functions as scheduled.
- Create Marketing Materials as needed

- Maintain friendly, cordial relations with all physicians and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive working relationships with physicians’ offices, customers, co-workers and managers.
