Patient Advocate

Shift flexibility between 8am-7pm Monday-Friday

• Builds trusted relationships with patients, prescribers, and stakeholders through proactive communication and timely execution.
• Manages relationships in compliance with laws, regulations, and industry standards, including HIPAA and insurance guidelines.
• Performs patient benefits verifications for major medical pharmacy plans.
• Completes Coverage Determination Summary forms for patient benefits.
• Coordinates prior authorizations based on payer guidelines and legal compliance.
• Reviews patient assistance enrollment forms to assess eligibility per SOPs and program guidelines.
• Determines patient eligibility based on program criteria.
• Communicates eligibility to patients, healthcare providers, and pharmacies.
• Conducts program welcome calls to patients.
• Performs post-Benefits Investigation calls to explain coverage options.
• Manages inbound calls (up to 50/day during peak season) using program-approved FAQs.
• Triages patients to internal or external resources as needed.
• Assists inbound callers by resolving queries or transferring to appropriate individuals.
• Reviews and identifies missing or incorrect information in service requests/applications
