About the Job
The Patient Transfer Coordinator directs the flow of patients into and out of Cabell Huntington Hospital, allowing for an exceptional customer service process for our referring physicians and facilities. For incoming patients who cannot be properly accommodated at Cabell Huntington, the Coordinator assists referring physicians and hospitals with securing patient care services at other facilities. When notified of a referral request, the Coordinator gathers information from referring facilities and arranges all aspects of patient acceptance. This process is completed in concert with referring and receiving physicians, the medical command center, the House Supervisor, and patient registration as appropriate. When notified of a need to transfer a patient from Cabell Huntington to another facility, the Coordinator provides information to the appropriate individuals and keeps transferring staff informed as the process is completed. In this role, the Patient Transfer Coordinator works in concert with referring and receiving physicians, the medical command center, the Nursing supervisor, patient registration, social services and case management, as appropriate. The Patient Transfer Coordinator serves as an advocate for customer service excellence. This goal is met through providing daily feedback on inpatients transferred from other facilities, assisting with marketing the many service lines available at Cabell Huntington Hospital, and striving to meet the tertiary transfer needs of physicians, staff and patients at referring facilities throughout the region. The shift will be determined by each hiring manager. The pay rate will be determined in accordance with the labor agreement. Emergency and/or critical care nursing experience with a clear understanding of appropriate patient triage. Appreciation of the importance of utilization review and/or quality assurance related activities Basic computer operation/keyboard experience Proven history of excellent customer service and communication skills. Special licenses, registration or certification: * Active, unrestricted registered nurse license in West Virginia * Preference will be given to those holding Certified Emergency Nurse and/or Critical Care Registered Nurse credentials * Preference will be given to those holding ACLS, BTLS/PHTLS, PALS/PEPP credentials Education: * Preference will be given to those holding a Bachelor' degree or higher Level and Type of Experience: * Minimum of two years experience in adult critical care and/or emergency nursing * Preference will be given to those with a proven history of excellent customer service Equal Opportunity Employer Minorities/Women/Protected Veterans/Disabled
